
Cancer+ How FrEDA Can Help Assess Patients in Cancer Pathways

This course consists of 5 lessons to learn about how to use Fr E D A in SystmOne, as an effective efficient tool to help proactively identify all people with frailty, dementia and any aged adult deemed likely to be within last year of life (end of life), deliver holistic assessments based on Comprehensive Geriatric Assessment (CGA) framework, and serve as an effective Personalised Care and Support Planning (PCSP) tool. Plus shows how Fr E D A can really be useful for us all to do this easily, connect teams to each other more efficiently, plus help follow the person journey to deliver more personalised and anticipatory care.
The 5 lesson elements within this course do not need to all be done at once and can be done separately in your own time at your own convenience. There is a short quiz at the end of the course to test your learning.
All videos will be accessible for future viewing and referencing once completed.
If working in a shared environment it is recommended that headphones are utilised to avoid any disruption and allow you to fully focus on the course.
It is estimated to take around 40 minutes to complete in one sitting. However, you can stop at any completed lesson, returning to the course at your convenience – where your current position will be stored – allowing you to progress the course until completed.

Find, Refer, Assess, Intervene, and Listen to according to evidence based best practice, as per MSE FRAIL plus framework, to help us improve people’s outcomes, improve their care coordination, their experience of care and to benefit our staff and services.
The course covers specific information on the evidence based “high impact” actions that can provide most benefit:
- Frailty, Dementia and End of life identification and recording in Fr E D A
- How to conduct and deliver a CGA for persons with frailty or dementia quickly and simply in Fr E D A
- Specific details on improving medication awareness, safer prescribing and polypharmacy issues for people with Frailty/Dementia
- Falls risk assessment
- End of life Advance Care planning and Anticipatory Palliative care for any aged adult who may be likely to be nearing toward end of life.
- Plus, how Fr E D A facilitates live multi-disciplinary working in the day to day- by including convenient quick live referral links to connect more efficiently to key local teams in your area.
